Age Groups: U6 for $140

Age Groups: U8 for $150

Age Groups: U10-U14 for $165

Sibling Discount of $10 off second registration
$20 discount if new uniform is not needed (Uniform from Fall)

* Non-refundable fees include: Referee and uniforms.

Sign‐up Requirements for both Returning & New Players

All forms will be provided at walk-in registration locations

1) Birth certificate or passport required only to verify birthdate (No copy needed)

2) Two completed and signed copies of the registration form

3) Liability card/disclosure form signed for player card/disclosure

4) For U14 & older players only: 1"x1-1/4" color photo of player’s head (no hats or sunglasses) NOTE: picture must fit in box to the right

Volunteers Needed!

MHYSL is looking for coaching staff & team parents to work with our youth. It is the volunteers within our community that help grow our youth soccer players into great athletes.
